The aim of this study was to optimize ultrasound investigation using dopplerography and radioisotope gamma-scintigraphy in the differential diagnosis of Graves disease and destructive thyrotoxicosis. We studied 68 patients with Graves disease and 36 ones with destructive thyrotoxicosis. Based upon the isotope scintigraphy, the uptake was significantly higher in patients with Graves disease (p<0,001). B-mode did not reveal fundamental differences in the structure of the thyroid gland in patients studied.

We estimated the significance of dopplerography in diagnostics of decompensated functional autonomy by. We examined 38 people. Autonomic nodules characterized by III and IV blood circulation types in the color Doppler scanning regime. Decrease of parenchyma perfusion was determined. Significant increase of speed parameters were recorded on the side with autonomic legion in comparison with contra lateral lobe. This method allows confirming functional autonomy in nodular goiter. However, the significant typing of the autonomic area by ultrasound examination is possible only in the case of single nodular.

The aim of the study is to optimize the ultrasound with Doppler and radionuclide gamma-scintigraphy in the differential diagnosis of Graves' disease (GD) and thyroid functional autonomy (FA). Altogether 106 persons (86 women and 20 men), of which 68 patients with Graves' disease and 38 patients with functional autonomy of the thyroid gland. According to the results of scintigraphy can distinguish a significant increase in the uptake of the isotope in patients with BG. In this case, despite the considerable variation in the total uptake Tc in a group of people with FA overwhelming share of the inclusion of the isotope belongs to the autonomy focus, parenchyma of the same isotope accumulates about 5 times smaller. In the color Doppler mapping in patients with Graves' disease in all cases there was an increase in vascularity with a uniform distribution of loci of blood flow. Patients with FA mode, the CDF in all autonomous nodes detected III and Type IV blood flow (61% and 39% respec tively), that is, in all cases involve some element of intran-odulyarnoy vascularization blood supply of the parenchyma in most cases was normal (I degree - 14 %) or reduced (0 degree - 83%). Thus, the basis of differential diagnosis by ultrasound are the data obtained in the CDF mode: for the FA is characterized by elements intranodular while low degree of vascularization of the parenchyma.
